An investigation that stops at operator error has found a person to blame and left the hazard in place. This course builds the discipline that gets past it. Delegates work through securing a scene and preserving evidence, interviewing witnesses whose memory is already changing, building a timeline from physical, documentary and testimonial evidence, and applying causal analysis that keeps going past the first plausible explanation. Substantial time goes to the report itself, because an investigation only changes anything through what is written down: findings that are evidenced, causes distinguished from contributing factors, and recommendations specific enough that somebody can be held to them.
