Human Factors Analysis for Incident Investigation

People do not come to work intending to cause an incident, and an investigation that concludes somebody was careless has usually described the outcome rather than explained it. This course covers analysing the human contribution properly. Delegates work through how attention, memory and decision making behave under fatigue, time pressure and high workload, the difference between a slip, a lapse, a mistake and a violation, and why the conditions that produce each are different. Task and workload analysis, interface and procedure design, and the organisational factors that shape behaviour are applied to real incident material. The course closes on writing human factors findings without producing an account that reads as blame.
