HAZOP and LOPA in Process Plants

A hazard and operability study is the main systematic defence against a major accident, and its quality depends almost entirely on how the session is run. This course covers running one properly. Delegates work through node selection, guide words and deviations, and the discipline of recording a cause, a consequence and a safeguard that are actually distinct from one another. Layer of protection analysis follows, taking the qualitative findings and testing whether the protection actually reduces risk to the tolerable level, including initiating event frequency, independent protection layers and the derivation of a required safety integrity level. The course covers facilitation, since the commonest failure is a session that agrees too quickly.
