Maintenance ManagementFailure Mode and Effects Analysis
Failure mode analysis is meant to find what can go wrong before it does, and it is commonly run as a workshop that fills a spreadsheet, scores everything moderate and changes nothing. This course covers running one that produces action. Delegates work through defining the item and its function precisely enough to analyse, identifying failure modes at the right level of detail, and effects traced through to the consequence somebody actually experiences. Severity, occurrence and detection rating are covered, including the calibration that stops every team scoring differently. Action priority and the decision on what to act on follow. Design and process variants are compared. The course closes on linking the analysis to maintenance tasks, controls and design change, and on keeping it current.
